Treatment of distal humeral fractures in the adult.

Fractures of the distal humerus in adults have unusual and difficult problems. Recently, we experienced three cases of distal humeral fractures in the adult treated by internal fixation. Two were males and one was a female. The average age was 37.3 years and the average duration of follow-up was 11.6 months. All cases were sustained in motor vehicle accidents. We employed the posterior approach using osteotomy of the olecranon and anterior transposition of the ulnar nerve, and fractures were fixed with two AO reconstruction plates on each epicondyle. Rigid fixation was achieved enough to allow early exercise by this method. Though one patient had varus deformity, the average elbow extension was -10.6° and flexion was 131.3°.We concluded that the anatomical reduction with rigid fixation gave best results.
