Severe myelosuppression following cisplatin and 5-FU infusion chemotherapy in the postoperative care of oral cancer. A case report.

A case of 54-year-old man with tongue cancer who received two courses of cisplatin and 5-FU infusion chemotherapy in the postoperative care was presented. The patient died one week after the course due to severe myelosuppression such as sudden leucopenia and thrombocytopenia, being thought the risk with chemotherapy was relatively low from clinical evaluation. We discussed causative factors on the drug-related death and obtained the following considerations:<BR>1) In the active chemotherapy, degree of the adverse reaction tends to severity with increase of the courses, and as frequent as possible blood examinations will be required.<BR>2) Chemotherapeutic agents by oral administration also considerably take part in manifestation of the adverse reaction. Then on the active chemotherapy, one month at least for pause of the drug will be needed.<BR>3) Though it has been considered that patients have adequately been relieved from adverse reactions by the improved admnistration of chemotherapeutic agents, even now risk has remained. Particularly, those of Japanese will have to be reduced in some measure in comparison with those of Caucasians.
