A case of hepatocellular carcinoma with pulmonary tumor emboli ; Double carrier of HBV and HTLV-I.

A 51-year-old man was admitted to our clinic with a chief complaint of sudden lumbago. Ultrasonogram and computed tomography revealed a large tumor in the liver and tumor emboli to the portal vein and the inferior vena cava. Twelve days after the admission, he complained of severe chest pain due to tumor emboli to the main trunk of pulmonary artery, documented by clinical, electrocrdiographic, radiologic and cardiac catheter studies. Lung perfusion scintigram showed a complete perfusion defect of the left lung. In addition, he was shown to be a double carrier of HB virus and HTLV-I (human T-cell leukemia virus type I) by immunological and hematological examinations. We suggested the possibilities that the occurence of hepatocellular carcinoma was related to HB virus and HTLV-I infection.
