It has been recommended that, during the treatment of obesity/metabolic syndrome, exercise duration should be progressively increased to approximately 300 min per week (60 min x 5 d/wk) (9). The objective of exercise therapy for the obese is to increase utilization of fatty acids from adipose tissue. The aim of the present study was to determine whether prolonged exercise is in fact effective for the treatment of obesity/metabolic syndrome by analyzing the relation between exercise duration and metabolic responses in healthy, untrained men. The mean respiratory exchange ratio (RER) during 90 min of moderate cycling exercise (mean heart rate = 117 ± 4 beats/min) was 0.87 ± 0.04 and the lactic acid concentrations were gradually decreased throughout the exercise trial. RER and plasma glucose levels also decreased gradually, while circulating free fatty acids (0.51 ± 0.16〜1.09 ± 0.45 mEq/l) and ketone bodies (67.3 ± 25.5〜210.4 ± 140.8 μmol/l) significantly increased during the 90-min exercise session. Considering the harmful consequences of post-exercise ketosis in obese patients, these results suggest that physical exercise aiming to reduce visceral fat and to improve insulin resistance should last for no longer than 60 min.
