Morphological Study on the Circle of Willis in Japanese

There have been many reports on the function and variation (VA) incidence of the circle of Willis. However, the reported incidences in Japanese people are too varied to provide a standard. The present study examines 2,665 unnatural or unexpected deaths that were autopsied at Tokyo Medical Examiner's Office, Tokyo Metropolitan Government and the Department of Legal Medicine, Kitasato University School of Medicine to determine the standard incidence of VA in the arteries forming the circle of Willis in Japanese people. One or more VAs were observed in 65.0%; the incidence of VA was 17.1% in the anterior cerebral artery, 14.8% in the anterior communicating artery, 36.1% in the posterior communicating artery, and 12.1% in the posterior cerebral artery. Comparison of 3,869 European and American and 2,009 Japanese people in the literature revealed no racial differences and thus yielded different results from those previously reported on Japanese people. VA depriving the collateral significance of the circle of Willis was less frequently noted in the anterior portion and more frequently observed in the posterior portion. Awareness of these findings is important when making clinical diagnosis or forensic determination.
