Does Epidural Analgesia Prolong Labor When Electively Induced?

To compare the effects of epidural and parenteral analgesia under elective labor induction on duration of labor, method of delivery, and neonatal outcome. A retrospective comparative study was performed between epidural and parenteral analgesia given to 806 nulliparas with uncomplicated term, singleton vertex fetuses that were admitted for elective induction of labor. Epidural analgesia (E group) was selected in 702 cases and parenteral analgesia (P group) in 104 cases on the basis of the day of the week. Perinatal events were compared between the two groups. Duration of labor (first stage, second stage, and induction-delivery time) was significantly longer in the E group than in the P group. A higher incidence of nonreassuring fetal heart rate patterns was noted in the E group. The incidence of C-sections for fetal distress was not significantly different. A significantly higher incidence of vacuum/forceps extraction was noted in the E group, but the incidence of C-sections due to dysfunctional labor was not significantly different. The neonatal outcome was favorable in both groups. Epidural analgesia seemed to prolong the first and second stages of labor compared to parenteral analgesia, although perinatal outcomes were comparable in both groups.
