A rare case of osteosarcoma arising from the sphenoid bone with distant metastases is presented. The patient was a 68-year-old man. The patient died 6 years after initial symptoms. At autopsy, a tumor in the sphenoid bone was found. The tumor invaded the right cavernous sinus, basal meninges, and intracranially. It extended to the spinal cord through the arachnoid space. Furthermore, distant metastases were seen in the lungs, liver, adrenal glands, urinary bladder, and vertebrae. The tumor in the sphenoid bone was composed of anaplastic cells forming irregular trabeculae of osteoid, bone, and cartilage. Osteoid or bone was not seen in the metastatic tumors, except for those in the vertebrae.
