The aim of this study was to investigate the difference in needs and demands for orthodontic treatment among orthodontic patients, dental students, and orthodontists. By using ten pre-treatment orthodontic cases selected based upon the patient distribut on of the orthodontic clinic in Tokyo Medical and Dental University, we have conducted a questionnaire survey asking the needs and demands for orthodontic treatment in these cases to Japanese orthodontic patients, dental students and senior orthodontists. While there was no significant difference in needs and demands in cases with severe crowding in all three groups, patients rated significantly lower needs and demands for orthodontic treatment in anterior deep bite, open bite, Class II , and Class III malocclusion. In most cases dental students rated a value in the middle of patients and orthodontists. These results indicate that even though patients are familiar with crowding, other malocclusions are not yet acknowledged as "to be treated", creating a discrepancy in needs and demands for orthodontic treatment between patients and orthodontists. Since higher level of dental education reduces this discrepancy, providing sufficient information and promotion on benefits of orthodontic treatment to the general society may improve the level of dental health.
