Acute Cerebellar Edema Following Intra-arterial Tissue Plasminogen Activator Thrombolysis for Basilar Artery Occlusion : Case Report

椎骨脳底動脈閉塞症例に対する急性期線溶療法の適応、特にtime windowについては不明の点が多い。その原因としては内頚動脈系にくらべ症状の発現時期がはっきりしない点があげられる。今回脳底動脈閉塞症例で発症10時間目で線溶療法を施行後小脳腫脹と急性水頭症をきたし、外減圧手術ならびに脳室トレナージ術を施行した61歳男性を報告した。脳底動脈閉塞症は再開通が得られなければ予後不良で発症12時間以内では再開通後症状の回復をみる例が多い。本例は線溶療法後遅発性に小脳腫脹をきたすことがあり適切な処置を要することを示唆するものである。
