Posterior Cerebral Artery Aneurysm Associated with Unilateral Internal Carotid Artery Agenesis : Case Report

内頚動脈欠損に伴う椎骨脳底動脈系の動脈瘤の報告は少ない. 最近, 一側内頚動脈欠損に合併した後大脳動脈瘤の一例を経験したので報告した. 症例: 38歳, 女性. 1994年5月30日, 突然の頭痛, 嘔気のため当科に入院した. 入院時, 項部硬直を認めたが, 意識は清明で, 脳局所症状はなかった. CTでは脚間槽に限局したくも膜下出血を認めた. 脳血管撮影では, 右後大脳動脈水平部-後交通動脈分岐部に動脈瘤を認めた. 右内頚動脈は起始部より欠損しており, 右中大脳動脈は右前大脳動脈および右後交通動脈を介して造影された. 側副血行が良好で, くも膜下出血も軽微であることから, 急性期に動脈瘤茎部クリッピングを行った. 7月24日, 軽度の左上肢の麻痺を残し退院した.
