Radiosurgery for Acoustic Neurinoma with Rapid Growth and Relatively High Staining Indexes for Proliferating Cell Nuclear Antigen and MIB-1 : Case Report

神経鞘腫が急速に増大することはまれである。我々は、3年の経過で急速に増大したと考えられる聴神経腫瘍例に対し、約70%を摘出し、残存腫瘍に対してはgamma knife治療を行い良好な結果を得た1例を報告した。患者は54歳の女性で、右小脳橋角部の膿瘍は17.3cm^3で、tumor doubling time (Td)は216日と算出された。手術後において残存腫瘍もTd 205日で増大したため1992年12月1日辺縁線量15Gyで照射した。摘出標本のPCNA、MIB-1の染色率は各4.1%、2.1%と神経鞘腫としてはかなり高かった。照射後2年半の経過で腫瘍は縮小し(1.4cm^3)、手術および照射治療による副作用は認められてない。gamma knife治療は急速な増大を示す神経鞘腫にも有効である。
