症例は50才男性。9カ月来の進行する右手の異常知覚と頭痛にて発症し、興奮状態となったため来院。MRIでは大孔部にGdで均一に濃染する腫瘍性病変を認め、小脳、延髄を圧迫していた。後頭下開頭と環椎椎弓切除を行い、cisterna magnaからcerebellomedullary cisternにかけて進展する腫瘍を認めた。腫瘍はMagendie孔の脈絡組織に付着しており、第四脳室への進展はなかった。病理診断は脈絡叢乳頭腫であった。Magendie孔の脈絡組織から発生し、脳室外ヘ進展した脈絡叢乳頭腫は稀と考えられ報告した。
