The mechanism of acupuncture analgesia by electrophysiological study : With special reference to dental clinic

Acupuncture analgesia has been utilized as useful technique in dental treatment and presents the therapeutic advantages of decreased bleeding during tooth-extraction and facilitated wound healing. However since the mechanism of acupuncture have not been completely explored, we attempted to elucidate the analgesic effect of acupuncture. Sensory (electrosensory) and pain thresholds were determined in the 48 subjects (patients and volunteers) with normal sensibility of gingival tissue and were recorded routine electroencephalogram (EEG), electromyogram (EMG) and evoked electromyogam. The mean sensory and pain threshold measured by electrical stimulation was 0.67 ± 0.19 V (mean ± SD) and 10.05 ± 2.02 V respectively. In acupuncture responders (effective cases), the sensory threshold elavated to 0.8-3.0 V and pain threshold elavated more than 20 V. The characteristic EEG findings in the responders were diffuse alpha pattern, and decreased appearance time of alpha-wave on the open-eye test. Reduced response of EMG activity during jaw-jerk reflex and undulated amolitude of H waves were observed under acupuncture analgesia. Conclusively, the electrophysiological findings suggested that generalized reversible hypofunction of the brainstem played an important role of the mechanism of acupuncture analgesic effect.
