<Original Article>Apoptosis in Relation to p53, WAF1, bcl-2 and Bax Expression in Cervical Intraepithelial Neoplasia

To elucidate the changes in cellular biological characteristics that occur in cervical carcinogenesis associated with high-risk human papillomavirus (HPV) infection, we examined the percentage of cells undergoing apoptosis (%Apo) and the proliferating cell population (%PCNA) in normal cervical tissue and cervical intraepithelial neoplasia (CIN), and determined correlations with cell populations expressing p53, WAF1, bcl-2 and Bax. Formalinfixed, paraffin-embedded sections were obtained from 30 women with a normal cervix, from 25 with low-grade CIN (L-CIN) and from 35 with high-grade CIN (H-CIN). 28 normal cervices negative for HPV 16/18 E6 oncoprotein and 56 L- and H-CINs positive for HPV 16/18 E6 oncoprotein were selected as a sample. Apoptotic cells were visualized by the TUNEL method, and both PCNA-positive cells and cells expressing the tumor suppressor gene or oncogene products were detected immunohistochemically. A significant decrease in %Apo was noted in the H-CIN group, whereas %PCNA was increased in both the L- and H-CIN groups. Compared to the normal cervix group, increased %WAF1 and %bc1-2 and decreased %p53 and %Bax were observed in the L- and/or H-CIN groups. The significant increase in %WAF1 and decrease in %p53 were noted in the H-CIN group compared to the L-CIN group.In all CIN specimens. %Apo was positively correlated with %p53 and %Bax,and %PCNA demonstrated a positive association with %WAF1. However, %Apo and %PCNA had no correlation with %bcl-2. We concluded that apoptosis is reduced by the functional inactivation of Bax and/or p53 and cell proliferation is elevated in line with the augmented expression of WAF1 during CIN progression.
