A 5 year-old Japanese boy developed the Guillain-Barre syndrome twice at an interval of two years. For the first episode, he received oral corticosteroid and recovered in a year. The second time, intravenous prednisolone (2 mg/kg/day) was given which increased to a dose of 2.4 mg/kg/day. After transitional improvement muscle weakness got worse and he complained of dyspnea. High does of intravenous methylprednisolone therapy was initiated, and then his respiratory function improved rapidly. He made an almost complete recovery in six months. This observation suggests that methylprednisolone pulse therapy may be beneficial in treating a patient with the Guillain-Barre syndrome.
