This paper describes the light, electron, fluorescence and electron-immunoperoxidase microscopic studies of the kidney from a thirteen-month old female baby with the nephrotic syndrome. Light microscopy revealed markedly increased mesangial matrix, intraglomerular fibrosis and moderately dilated proximal tubules. By electron microscopy, mesangial increasing new thin basement membrane and basement membrane thickness of subendothelium were shown. Fluorescence microscopy revealed IgM, IgG, C3 globulins and fibriogen. In electron immunoperoxidase method, Iocalization of IgM, IgG, C3 and fibrinogen was observed. A diagnosis of infantile diffuse mesangial sclerosis was made from the clinical and histological data. These observations suggest that actions of both immunological and coagulative systems might cause this disease.
