Background: The adjunctive role of dominant frequency (DF) mapping during complex fractionated atrial electrogram (CFAE) ablation after pulmonary vein isolation (PVI) for atrial fibrillation (AF) has not been clarified. Methods: After PVI, CFAE-mean and DF were measured during AF by means of NavX mapping system (n=17 persistent AF patients). Results: Regional distribution of the maximum DF sites and the shortest CFAE-mean sites were located at adjacent sites that were mainly located at the orifice of posterior left atrium, left atrial appendage, septum and mitral annulus. However, shortest CFAE-mean sites were observed 1–2 cm outside the DFmax sites, i.e., The DFmax (8.5±1.1 Hz) site represented 114.3±59.8 ms of CFAE-mean, 1 cm from DFmax site (6.4±0.9 Hz) was 81.5±24.3 ms of CFAE-mean, and 2 cm from DFmax (6.4±0.8 Hz) was 83.0±23.4 ms of CFAE-mean. Conclusions: The spatial distribution of fractionated activity was located adjacent to the DFmax site, but continuous CFAEs extended from the center of DFmax.
