原発不明頸部転移性腫瘍10例の検討

Ten cases of primary unknown metastatic cervical tumor were studied. Histopathological examination of these tumors revealed adenocarcinoma in 5 cases, squamous cell carcinoma in 3 cases, adenoid cystic carcinoma in 1 case and malignant thymoma in 1 case. The primary sites, which were determined at the time of autopsy, were lung, thymus, cervical esophagus, pancreas and minor salivary gland of lower lip.Radiotherapy was performed in 8 cases and surgery was performed in 3 cases, because these cases were advanced and inoperable. Radiation doses ranged from 34Gy to 61Gy (mean 51.6Gy). The radiation field was restricted to the cervical tumor. Systemic chemotherapy was given in 4 cases combined with surgery.The prognosis of these cases was poor. The longest survival duration was 59 months and the 3-year cummulative survival rate was about 40%.Causes of death were massive bleeding from cervical tumor in 2 cases, sepsis, uremia, respiratory failure and unknown in 1 case each. Control of cervical tumor is important for improving the prognosis.
