We treated five cases of open fracture of the ankle part with soft tissue damage between 1980 and 1986.All cases had trouble with the circulation of the blood, therefore, three cases were treated with anastomoses of the blood vessels and four cases with skin grafts.After injury, scrubbing, washing and débridement are absolutely necessary.We do anastomoses of the blood vessels actively by microsurgery, and do free composite grafts of skin and fat for skin defect. This method preserves subcutaneous vascular network and is very useful for skin graft over the joint, because the graft skin develops no contracture nor wrinkling.Furthermore, we apply intermittent compression with milking cuff to make adaptation to the recipient bed successful.
