Dermatomal Somatosensory Evoked Potentials(DSEP) in Lumbar Nerve Roots.

It has been reported that dermatomel somatosensory evoked potentials (DSEP) are reliable method for the evaluation of nerve function in spinal disorders. The aim of this experiment is to investigate the sensitivity and selectivity of DSEP and peripheral antidromic sensory potentials (ASR) by dorsal rhizotomy at the cauda equina, using an anesthetized monkey. Nerve roots at L1-S2 were exposed and identfied at each level. DSEPs were recorded from the cervical spine and scalp (C3, C4, Cz), by stimulating the dermatomes of the medial side of gastrocnemius, the Ist and Vth toe. SEP was elicited by stimulating the sciatic nerve. ASR was recorded from peripheral dermatomes and the sciatic nerve by T10 stimulation. Dorsal rhizotomy selectively demonstrated the decrease in potential of the root lesion, and high sensitivity in DSEP and ASR at peripheral dermatomes. However, SEP and antisensory response at the sciatic nerve were less sensitive to dorsal rhizotomy. The usefulness of DSEP and ASR at peripheral dermatomes suggested the possibility of clinical application for diagnosis of radiculopathies.
