Chemotherapy.A case of advanced stage of malignant melanoma in which CPT-11 ( Camptothecin derivative ) was effective.

The patient was a 37-year-old housewife, with no reported family history or personal history.Twenty years previously, she found dark-brown pigmentation on her lower eyelids. One year prior to its excision in June, 1985, the lesion exhibited partial elevation and darkening. It was subsequently diagnosed as nodular melanoma. In April of 1989, she had bloody sputum and an examination of her head and neck revealed a hemorrhagic mass on the epipharynx. Abdominal CT scanning revealed metastasis in the form of intraabdominal lymph nodes. In June of 1989, 80mg/m2 of CPT-11 in 500ml of 5% glucose was infused intravenously. One course of the treatment consisted of one infusion a week for approximately 4 weeks. A total of 5 courses were administered. During the second course, the size of the mass was reduced. A partial response was obtained after the fourth course, and by the fifth course the lesion nearly disappeared. As of December 1991, the patient has been in good health and there has been no recurrence of the mass. Her only complaint was of mild nausea and vomiting 5-6 hours after infusion. Hematological finding revealed no abnormalities.
