A case of severe acute hepato-renal damage caused by repeated lacquer thinner sniffing in a 17-year-old male student was reported. He had inhalated lacquer thinner with his fellows almost every day for two months, and then, feeling general malaise, nausea, vomitting, anorexia, he consulted the neighboring doctor, who pointed out severe liver damage (GOT 9080 U/l, GPT 6600 U/l, LDH 10305 U/l) and referred to our hospital. Physical examination on admission revealed no definite abnormalities except for jaundice and hepatomegaly. Liver function tests showed severe liver damage (T. Bil. 4.5mg/dl, GOT 1740 U/l, GPT 3828 U/l, HPT 50%) and renal function tests demonstrated renal failure (BUN 61.4mg/dl). After admission and receiving medical treatment, hepato-renal damage improved quickly and laboratory data had been normalized in a month. Liver biopsy specimens revealed hepatocytes necrosis and congestion around the central vein. No inflammatory cell infiltration was seen both in Glisson sheath and in lobules. Toluene, a major component and trichlorethylene, a minor one of lacquer thinner are considered to be probable cause of hepato-renal damage in this case.
