Since 1978, in cooperation with Osaka Cancer Research Foundation, we have conducted mass screening programs for colorectal cancer by fecal occult blood test. Recently immunological method, being very specific in detecting human hemoglobin, employing reversed passived hemagglutination reaction (RPHA) has been developed. In this study, We attempted to establish a new economical RPHA method for detection of fecal occult blood from mixed extract of 3-day specimens. We have evaluated the validity of applying this method to the mass screening over nine months period. 186 out of 5, 897 individuals were positive with a positive rate of 3.2 % and a positive predictive value of 9.7 %. 10 persons had colorectal cancer, of whom 6 were in early stage. This new method was found to be more effective for a mass screening and appears to have high diagnostic value.
