The hepatocyte-extracellular matrix interaction is biologically relevant to regulate liver-specific genes by activating hepatic transcription factors, hepatocyte nuclear factor-1 and -4, and a basement membrane matrix, Engelbreth-Holm-Swarm (EHS) gel was most effective for maintenance of the differentiated phenotype of hepatocytes in cell culture. A bioartificial liver support device which is combined with a hollow fibermodule and hepatocytes entrapped in EHS gel improved hemodynamic stability, increased blood bicarbonate levels, and decreased plasma ammonia levels in pigs with ischemic liver failure, suggesting that this system could have potential advantages for clinical use in patients with fulminant hepatic failure.
