Gastrointestinal stromal tumor of the stomach treated by urgent laparoscopic partial gastrectomy in an elderly woman : Report of a case

• Gastrointestinal stromal tumors are rare lesions arising from connective tissue anywhere along the length of the gut. Treatment of a gastrointestinal stromal tumor of the stomach by laparoscopic partial gastric resection is reported. An 85-year-old woman was admitted to our hospital with hematemesis. Esophagogastroduodenoscopy (EGD) showed bleeding from the central ulceration of a submucosal tumor of the gastric fornix. Abdominal computed tomography showed a 5-cm tumor in the anterior wall of the stomach with a clear margin and central low-density area. The tumor was resected by laparoscopic partial gastrectomy with endoscopic guidance. A total gastrectomy was not performed because the patient could tolerate only a minimally invasive procedure. The tumor showed immunoreactivity for KIT, CD34, and neuron-specific enolase, but not for α-smooth muscle actin, w-actin, or S-100 protein. The final diagnosis was a gastric gastrointestinal stromal tumor. The postoperative course was uneventful, and the patient was discharged on the 11th postoperative day. She was asymptomatic when last examined three years after surgery. Laparoscopic surgery is an adequate strategy for gastric gastrointestinal stromal tumor. This technique is curative and safe, and is especially appropriate in the elderly.
