Lateral Humeral Epicondylitis : Results of Conservative and Surgical Treatments

A survey was conducted on 155 patients with lateral humeral epicondylitis who had been treated conservatively and/or surgically. With three-step conservative treatment, clinical symptoms were improved in 139 cases. No statistical significance was noted between the recovery rate and patient's background factors such as age, involved site, sex, duration of clinical symptoms, and causes (mean follow-up : 5 years). In 10 out of 16 patients who did not respond to conservative treatment, surgical intervention (Boyd's procedure) was applied. All cases but one showed improved clinical symptoms within an average of 4 months after surgery (mean follow-up: 2.5 years). Macroscopic observation during surgery revealed induration of the fascia (2 joints), induration of the enthesis (4 joints), impingement of the synovial fringe (2 joints), proliferation of the synovial tissue (2 joints), and rupture of the annular ligament (2 joints). Histologically, a thick arrangement of the fascial tissue (2 joints), degeneration or proliferation of the enthesis (7 joints), mild degeneration or proliferation of the synovial tissue (6 joints), and degeneration of the annular ligament (2 joints) were observed.
