The authors encountered nine cases of painful dislocation of the extensor tendons in the dorsal side of the MP joint of the hand. In seven cases in which dislocation occurred in the middle finger, open reduction was performed and the results were generally satisfactory. During the operation, rupture of the intertendinous fascia was noted in all seven cases, suggesting that this fascia plays an important role in this pathological condition. In the remaining 2 cases, an incision of the abnormal connection band (case with little finger) and conservative treatment (case with index finger) were indicated respectively. To clarify the pathomechanism in the development of these conditions, experiments were performed using an amputated hand. The authors' findings indicated that tendon dislocation is anatomically more likely to occur in the middle finger than in the index, ring, or little fingers.
