91 reconstructions in 80 patients in the lower abdomen (bladder region), buttocks, groin, perineum and their adjacent regions were experienced for the past 15 years with satisfactory results except 4 ones. Mesh skin grafts, cutaneous flaps and musculocutaneous flaps were applied in 51, 19 and 20 regions respectively. A total leg flap was utilized in one patient who sustained a severe degloving injury. A wide or en-bloc excision of the tumor or diseased lesion, or a thorough debridement of the ulcerated wound, based on the preoperative acurate plan, was the most important maneuver, associated with the adequate urinary and/or fecal control. Reconstructive techniques in each surgical defect or chronic wound should be decided according to a reconstructive ladder-progressing from simple to more complex procedures. The second or salvage operative procedure should always be in due consideration of minor to irreparable results of the first operation. A long-term follow-up is also necessary for the protection against the complicated postoperative problems.
