A longitudinal abdominal bipedicle flap was used for reconstruction of a central abdominal wall defect, caused by a breakdown after two successive median laparotomies in a 71-year-old man, without fascia repair. This flap is simple and relatively safe because it includes both the superficial epigastric vessels and the perforating cutaneous branches of the superior epigastric vessels. The author regards it as the method of choice in the repair of moderate to major defects of the central abdominal wall under certain circumstances.
