The treatment patients with insomnia caused by pollakiuria with organic brain disorder is discussed. Two patients who had undergone brain aneurism rupture operation complained of "sleep disorder unspecified : diagnostic code G47.9" by the international statistical classification of disease and related health problem 10. They were administered mild benzodiazepine and propiverine hydrochloride, which is the medicine for pollakiuria instead of hypnotics. The patients could thereafter sleep very well without side effects caused by hypnotics such as speech disturbance, urine incontinence and ataxia. In the treatment of insomnia with an organic brain disorder, physicians need to pay attention to the presence of pollakiuria during the nighttime to avoid hypnotic side effects as much as possible, rather than using sleeping pills right away. Physicians should treat insomnia patients with attention psychiatric points of view, considering urology, organic brain disorder and psychiatry, especially in elderly patients.
