Four patients with Restless legs syndrome (RLS) are described. Patients were initially misdiagnosed as having either acathisia, insomnia, depression, pain disorder, or panic disorder. All the patients were treated based on each initial diagnosis. The initial treatments had no effect at all. Thus the patients were introduced to our hospital. Sufficient interviews and examinations clarified the existence of a sensation that patients were unable to express except to call it "restlessness", and that was related not only to the feet but also to the whole body. Thereafter patients were diagnosed with RLS only after sufficient interviews and examinations. Clonazepam was administered to all the patients, and had a dramatic effect after several days. The examination for the existence of RLS is very important for diagnosis. RLS is not wellknown in Japan but we should suspect a large incidence of RLS. Therefore, RLS should be known to every physician and psychiatrist.
