A case of REM sleep behavior disorder during puberty

We encountered a 20-year-old female with REM sleep behavior disorder that developed during puberty. The main complaint was nocturnal shouting. At the age of 15 years she began to shout while dreaming, and the content of shouting was consistent with that of the dream. At the age of 18 years, she also developed behavior consistent with the content of the dream. At the age of 20 years, she was admitted to the medical department of our hospital due to SLE, and subsequently referred to our psychiatric department due to development of nocturnal abnormal behavior. Concerning family history, her aunt had nocturnal abnormal behavior, and her family members had Hashimoto's disease. Diagnostic imaging of the head, routine neurophysiological examination, and blood examination (excepting SLE, lupus nephrtis) showed no abnormalities. Based on findings of polysomnography (PSG), a diagnosis of idiopathic REM sleep behavior disorder was made. Since the involvement of genetic factors in idiopathic REM sleep behavior has been suggested, the home environment in which she grew up surrounded by family members with Hashimoto's disease may have been a precipitating factor.
