Cystic Lesion at the Foramen Magnum Disseminated from a Pituitary Adenoma : Case Report

症例は38歳男性、19年前に下垂体腺腫にて腫瘍摘出術を施行した。今回歩行障害、嘆下障害を来たし頭部CT, MRIにて大孔部にcystic tumorを認めた。transchondylar approachにて腫瘍摘出し症状は軽快した。病理所見は典型的下垂体腺腫でMIB-1、p53染色陰性であり、明らかな悪性所見は認められなかった。遠隔転移をきたす下垂体腺腫はpituitary carcinomaとして知られているが、本症例のように原発腫瘍の診断から10年以上経過した後に転移を認めるケースもあり悪性所見を示さない下垂体腺腫においても遠隔部転移を考慮にいれ注意深くfollow-upする必要があると思われる。
