Surgical Treatment of Intractable Epilepsy Originating from the Primary Sensory Area of the Hand : Case Report

症例は14才女性。1才より3才まで有熱時痙攣が数回あり。6才より左手の知覚発作と二次性全般発作が出現し、薬物で発作抑制が困難であった。発作は左手のしびれ感に続く左手首のびくつきで始まり、右上肢屈曲挙上、左上肢伸展の強直姿位をとり全般性間代性痙攣に至る。神経学的には正常で、和田テストで左言語優位だった。MRIでは右中心後回に、TI低信号・T2高信号病変を認め、MEGで正中神経・尺骨神経誘発磁界の間にあることが示された。頭蓋内発作時脳波で、発作起始は病巣近傍の中心後回で、皮質刺激により手の一次知覚野であることが示された。手術は病巣切除と周辺皮質へのMSTで、永続的神経脱落症状なく発作の完全消失を得た。
