Long-term Evaluation of Radiation-induced Brain Damage by Serial Magnetic Resonance Imaging

遅発性放射線脳障害の発生および経時的な変化をMRIにて検討した. 対象は, 放射線照射後平均45ヶ月経過観察した8症例である. 放射線脳障害は照射後3〜30ヶ月(平均12.8ヶ月)目にGd造影T1強調画像で増強病変として出現した. 増強病変は, 3〜23ヶ月(平均10.3ヶ月)にわたり増加し続けたが, 長期生存4例において, 放射線脳障害発生後12〜35ヶ月(平均19.3ヶ月)後に増強病変の大きさが縮小するという変化を示した. このうち2例では, 病巣の縮小後それぞれ8および9ヶ月目に再燃した. 経時的MRIにおける遅発性放射線脳障害の変化は, 照射後1年以上は増悪を続け, 多くはその後消退するが, 中には再燃する場合もある.
