Phase II Study of DTIC ACNU, and Vincristine Combination Chemotherapy for Supratentorial Malignant Astrocytomas

ニトロソウレア系抗腫瘍剤の抗腫瘍効果増強と耐性抑制を目的にDTICの前投与を行い, その有効性を検討した. 対象は1988年7月から1993年3月に経験したテント上悪性グリオーマ32例. 13例がanaplastic astrocytoma, 19例がglioblatoma multiformeである. 全例で手術後, 50〜65Gyの放射線照射を行った. 化学療法のプロトコールは, DTIC 1mg/kgをday 1より5日間連続, ACNU 2mg/kgをday 5, vincristine 0.02mg/kgをday 1と15に静注し, 5週間毎に繰り返した. 結果はCR1例(3%), PR12例(38%)で, 生存期間中央値は18ヶ月, 再発までの期間の中央値は11ヶ月であった. この結果は, 我々の以前の治療成績に比べ有意差を得られず, DTIC 1mg/kgの5日間連続投与ではニトロソウレア系抗腫瘍剤耐性の改善には至らないといえる.
