Cervical Osteoplastic Laminotomy Using Autogenous Bone : Technical Note

棘突起からの自家骨を用いた骨形成的頸椎椎弓切除術を考案した。両側の第5から第8頸神経根に対するforaminotomyを行う。第3頸椎から第7頸椎までの関節に沿って溝を作り、神経根症の強い側にて、椎弓のosteotomyを行う。完全に切除していない側を支点として、第3から第7頸椎までの椎弓を一塊として持ち上げる。減圧が不十分な部位があれば追加する。あらかじめ切除して保存しておいた第6及び第7頸椎の棘突起を、持ち上げた椎弓塊とosteotomyをした側の関節の間にはさみ、絹糸にて固定する。この方法では、硬膜外の瘢痕形成や異物による合併症がない。金属による artifactが無く、高価な材料は不要で、頸髄や頸神経根に対して十分な減圧が可能である。
