Sigmoid Sinus Dural Arteriovenous Malformation Resulting from Jugular Foramen Schwannoma : Case Report

頸静脈孔神経鞘腫（JFS）によるS状静脈洞の閉塞と同部の硬膜動静脈奇形（DAVM）を併発した1例を報告する。症例は69歳男性。数年前から右難聴を認め、最近さらに眩暈と拍動性耳鳴が出現した。頭蓋内外に進展する右頸静脈孔腫瘍、腫瘍によると考えられる右頸静脈球部からS状静脈洞の閉塞と右S状静脈洞のDAVMを認めた。DAVMの経動脈的塞栓後、腫瘍とDAVMを摘出、腫瘍の病理診断は神経鞘腫であった。拍動性耳鳴は塞栓術後軽快した。術中JFSはS状静脈洞を充満・閉塞し、小脳橋角部で聴神経を圧迫していることが確認された。症候学的経過や術中所見から、JFSによる静脈洞の閉塞がDAVM発生の原因と考えられた。
