FETAL ACTIVITY DETERMlNATION AND NEOSYNEPHRlNE TEST FOR EVALUATION OF FETAL WELL-BElNG IN HIGH RISK PREGNANCIES

当科においては従来より産科特殊外来を設げ,特にhigh risk pfegnancy妊婦に対して,各種検査を組み合わせて胎児評価をすることにより周産期異常発生予知のスクリーニソグに努めている.今回197例の画high risk pfegnancy妊婦を対象として,non sttressed FHR monitoringとしてのfetal activity acceleration determinatbn(FAD)と負荷試験としてネオシネフリンテストの2つの検査により周産期異常の発生頻度についてretrospectiveに解析を試みた.単独検査による周産期異常発生頻度をみると,FAD197例中FAD異常は28例,そのうち胎児仮死は14例(50%),一方169例が正常で,そのうち新生児仮死3例(1.8%)が発生した.ネオシネフリンテスト62例中異常は6例で胎児仮死は4例(66.7%),新生児仮死2例(50%),一方ネオシネフリソ正常例56例のうちでは新生児仮死が1例発生したのみであつた.両検査を組み合わせると,両検査とも正常な51例では新生児仮死1例のみ,逆に両検杏とも異常は2例あり,2例とも胎児仮死及ぴ新生児仮死が発生した.以上の結果,FAD及ぴネオシネフリンテストは的中率の高さ,安全性よりみてhigh risk pfegnancyにおげる胎児評価,周産期異常発生予知のスクリーュングに極めて有用であることが認められた.
