Clinical Characteristics of J-Wave in the Healthy Subjects

<B>Background:</B> Recently, the presence of J-wave has been reported to be associated with idiopathic ventricular fibrillation (IVF). This variant has been found in 3 to 13% of the general population. However, the clinical characteristics of J-wave remain unknown. <B>Methods and Results:</B> (1) The consecutive 6860 12-lead ECGs (3805 men, 1–99 years) recorded in our hospital were studied. J-wave was defined as terminal QRS notching with amplitude of ≥0.05 mV in any leads. J-wave was detected in 10.8% in all subjects (12.0% in men and 9.3% in women) and observed most prevalently in the inferior leads (84%). (2) We investigated the circadian variation of J-wave using Holter ECG and the relationship between the J-wave amplitude (J-amp) and heart rate and heart rate variability in 16 healthy subjects with J-wave and 3 IVF patients. J-amp had a circadian variation, higher at night than during the day, in both groups. However, J-amp was significantly higher in IVF than control (p<0.0001). J-amp showed a significant negative correlation with HR and LF/HF and significant positive correlation with HF in both groups. (3) We studied 50 patients with distinct false tendon (FT) detected by echocardiography (25 men, 60.0±15.6 years) and 80 age- and sex-matched subjects without FT. The subjects with FT were more prevalent of J-waves than those without FT (61% vs. 25%, p<0.0005). <B>Conclusion:</B> These observations may propose important information for management of J-wave in the general population.
