A Simple Strategy to Prevent Esophageal Injuries: Pulmonary Vein Isolation Avoiding the Esophagus Visualized by Carto-Merge System

<B>Background:</B> Left atrioesophageal fistula is a fatal complication of atrial fibrillation ablation. To date, no standard approach has been established to prevent this complication. <B>Methods and Result:</B> This study included 21 patients. Pulmonary vein isolation (PVI) was performed under the guidance of esophageal visualization using CARTO-Merge system. The esophageal injuries were then evaluated on esophageal endoscopy. All radiofrequency applications on the left atrial posterior wall were limited to 20 W for 15–20 s using irrigation catheters, and were delivered more than 2 mm away from the esophageal edge of the merged CARTO image. Esophageal endoscopy was performed 2 days after PVI. No abnormal findings were observed in any patients. <B>Conclusion:</B> Longitudinal ablation on the left atrial posterior wall based on the esophageal visualization obtained by CARTO-Merge system could prevent esophageal injuries.
