The Relationship between Local Conduction Velocity and Amplitude in Right Atrail Flutter Reentry

The relationship between conduction velocity (CV) and amplitude in atrial flutter (AFL) remains unclear. We analyzed 26 AFL reentries in the right atrium (Group T: 10 typical AFLs; Group I: 10 incisional AFLs; Group NI: 6 non-incisional AFLs), focusing on CV and bipolar electrogram amplitude using CARTO system. A total of 194 points (7.4±1.5 points/circuit/patient) being considered to be on the circuit were selected based on an isochronal and propagation map. Tachycardia cycle length (TCL) of Group T, Group I and Group NI was 230.5±20.5 msec, 275.7±51.3 msec and 281.8±31.0 msec, respectively (P=0.018, Group T vs Group NI). Circuit length was significantly longer in Group T (123.1±34.7 mm) than in Group I (185.4±20.4 mm) and Group NI (136.6±41.1 mm) (both P<0.05). Mean CV along the circuit was significantly higher in Group T (0.805±0.069 m/sec) than in Group I (0.512±0.190 m/sec) and Group NI (0.436±0.109 m/sec) (both P<0.01). The area under a curve of natural logarithm of amplitude was significantly higher in Group T (−379.2±162.3) than in Group I (134.0±61.4) and Group NI (−284.7±301.2) (both P<0.01). In conclusion, the lower the amplitude was, the slower the CV was, and consequently, the circuit length was reduced.
