Sound isolation of the round window in the tympanoplasty type IV was achieved by an air-containing silicone ball for ears with Eustachian tube dysfunction.The chief aim of hearing improvement in tympanoplasty type IV is to create a relative sound pressure difference between the round window and the oval window by the new tympanic membrane, which encloses the lower part of the tympanum and protects the round window from direct sound energy.Theoretically this round window protection effect can be achived by an air-containing silicone ball placed on the round window, thus offering 20dB Air-Bone-Gap hearing.Eighteen ears were operated on using this method and in 4 (22.2%) of ears successful results with theoretical hearing acuity of 20±5dB Air-Bone-Gaps were obtained.Hearing levels in two ears dropped from the nearly theoretical levels to the level of the radically operated ears after rejection of the silicone balls.These 6 ears showed that sound isolation of the round window by the silicone ball allows for improvement in hearing in ears with a destroyed Eustachian tube, such as occurs after radical mastoidectomy. The material and the shape can be improved even more with further investigation.
