In this study, the effect of Sai-rei-to on the mucosal pathology of endotoxin-induced otitis media with effusion (OME) was studied. The lipopolysaccharide (LPS) from Klebsiella pneumoniae (0.2ml of 10μg/ml) was injeceted into the tympanic cavities of guinea pigs. Such intratympanic inoculation of LPS induced a variety of mucociliary disorders, which included prolonged middle ear effusions, reduced ciliary activity as well as deteriorated mucociliary clearance of the tubotympanum, pathological mucus blanket and ciliated cells, and sloughing of ciliated cells. Few recovery signs were seen in the tubotympanum as late as 6 days postinoculation of LPS.Animals treated with 120mg/kg/day of Sai-rei-to after the intratympanic inoculation of LPS showed a moderate recovery from the above mentioned mucociliary disorders. On the other hand, animals treated with 600mg/kg/day of Sai-rei-to demonstrated quick recovery from the above mentioned disorders. Normal mucociliary function and epithelial morphology were observed in the tubotympanum by the 6th day postinoculation of LPS. Our study suggests that clinical application of Sai-rei-to could be most probably useful in the management of patients with OME. Also our study demonstrated that the major mechanism by which Sai-rei-to relieves the middle ear from OME is excitation of ciliary activity in vivo as well as in vitro.
