Eight cases of carcinoma of the external auditory canal were classified into three groups according to the extent of the malignancy:Group I. Tumor limited to the external auditory canal.Subgroup A: no invasion of bone is suggested radiologically; tumor is located at some distance from the tympanic membrane (three cases).Subgroup B: no invasion of bone is suggested radiologically; tumor is close to or in contact with the tympanic membrane (two cases).Group II. The tumor invades anteriorly the parotid gland and the temporomandibular joint, although the middle ear and mastoid air cells are not involved (one case). Group III, The malignancy has already spread to the middle ear and/or mastoid air cells (three cases).Both Groups I and II were treated surgically to eradicate the disease, while a combination of surgery and irradiation was performed in Group III.In Group I, tumor recurred in only one patient (subgroup A). This case was reclassified in Group II at the time of recurrence, and the tumor was removed surgically. One year later there was no evidence of recurrence. Two patients in Group III died within a year and the third remains well 14 months after discharge. In conclusion, successful management of carcinoma of the external auditory canal depends on early diagnosis, correct evaluation of the extent of the malignancy and adequate surgery based on correct evaluation.
