A method on spinal osteotomy for the correction of thoraco-lumbar kyphosis a case report of pedicle osteotomy.

To surgically correct severe fixed kyphotic deformity associated with ankylosing spondylitis in the abvanced stage, usually wedge osteotomy has been performed from the posterior segment of thoraco-lumbar region. However, this technique has often accompanied with mortal or non-mortal complications due to angular deformity and over correction. Pedicle osteotomy in addition to wedge osteotomy of the laminae and articular facets was performed for so severe fixed thoraco-lumbar kyphosis with ankylosing spondylitis as the patient was very difficult to look at the front. The result of correction was satisfactory and lumbar lordotic angle was 42 degrees by Wiltse' method (-3 degrees before operation). The lumbar lordosis was morphological and cosmetic without angular deformity. Our technique seemed to be valuable and relatively safe to avoid complications.
