Dialysis patients are known to be immunodeficient. To study the susceptibility to miliary tuber culosis and the prognosis of the disease in dialysis patients, an epidemiological investigation was done.
The subjects consisted of 7, 274 dialysis patients including 150 cases complicated with tuberculosis. Among them the tuberculous lesions were documented in 137 cases. Nine males and six females suf fered from and seven males and six females died of miliary tuberculosis.
Incidence of miliary tuberculosis was 170 and 210 per 105 persons. year in male and female, respectively, among dialysis patients. These figures, were 548 fold in males and 778 fold in females higher than those in the age and sex matched general population, respectively. Incidence of miliary tuberculosis by age in dialysis patients showed a high plateau between 20 and 69 of age with a peak in 50s against the gradual increase of the incidence with the age in the general population.
Mortality rate from miliary tuberculosis was 132 and 210 per 105 persons-year in male and female, respectively, among dialysis patients. The rates were 1, 467 fold in males and 2, 625 fold in females higher than those in the general population. Mortality rate by age also showed a similar pattern to the incidence by age with a peak at 50s in dialysis patients against a constantly increasing pattern with age in the general population.
The fatality rate was 78% in males and 100% in females among dialysis patients in contrast to 30% in both sexes among the general population.
The mean duration of the disease among died cases was only 22 days in males and 25 days in females in the dialysis group.
Miliary tuberculosis occupied 10.7% in males all 11.3% in females of all tubercuosis among dialysis patients, whereas they were 0.2% in males and 0.34% in females in the general populatlion. Miliary tuberculosis occupied 44% in males and 60% in females of all deaths from tuberculosis among dialysis patients, which were much higher than 0.57% in males and 1.51% in females among the general population.
It was first elucidated that the dialysis patients readily develop miliary tuberculosis with rapid and fatal prognosis.
