A case of cystadenoma of the liver with marked elevation of tumor marker CEA, CA19-9 and SPan-1.

A case of cystadenoma of the liver is reported. A 71-year-old woman was followed up for postoperative course after operation for esophageal varices. Examination revealed her to have a monolocular cystic lesion of the liver and high serum levels of CEA, CA19-9 and SPan-1. No abnormal findings suggestive of malignancy were obtained. Cytology of the cystic fluid revealed no malignant cells, but the levels of CEA, CA19-9 and SPan-1 in the cystic fluid were abnormally high. Histological examination of needle biopsy specimens yielded findings compatible with the diagnosis of cystadenoma. Those tumor markers were proved to be localized on epithelial cells immunohistochemically. Operative therapy was withheld in view of her poor functioning liver. Now, 20 months after diagnosis, the patient is well with high serum levels of CEA, CA19-9 and SPan-1.<BR>Cystadenoma of the liver is rare, only 15 cases having been reported so far in Japan. According to Japanese literature, cystic fluid was positive for CEA and CA19-9 in all cases. Tumor markers in the serum and cystic fluid do not seem to be diagnostic of cystic lesion of the liver. Diagnosis should be made only on the basis of a careful scrutiny of diagnostic images and biopsy findings.
