At present the diagnosis of inflammatory bowel disease (I. B. D.) by examinations of X-ray, endoscopy and conventional biopsy is not fully satisfactory. For the better diagnosis of them, studies were made on 69 of ulcerative colitis (U. C.), 20 of Crohn's disease and 19 of intestinal tuberculosis. In cases of U. C., inflammatory polyposis was found prominent in cases with 1 to 3 months' history from the initial attack and more frequently seen in severe cases, and histological examination of the resected specimens or specimens obtained by polypectomy revealed marked inflammation of the mucosa with loss of the muscularis mucosae (m. m.). In cases of Crohn's disease, cobblestone appearance and inflammatory polyposis were histologically similar, and histological examination of the resected specimens and polypectomized specimens showed mild inflammation of the mucosa and the m. m. remained intact. In cases of intestinal tuberculosis, inflammatory polyps were found multiple, and the resected or polypectomized specimens revealed moderate inflammatory changes of the mucosal epithelium and frequent disappearance of the m. m.. From the present studies, it is concluded that the polypectomy examinations could be equivalent to the examinations of the resected specimens in yielding informations to make more accurate differential diagnosis of I. B. D..
