Malignant Tumors of the Anal Region

During the 11 years period 1962-1972, 29 cases of malignant tumors of the anus and anal canal were treated in our hospital. These were 4 cases of ordinary squamous cell cacinomas, 3 cases of transitional cell carcinomas, 4 cases of malignant melanomas, a case of Paget's disease and 17 cases of adenocarcinomas.<BR>The case of Paget's disease was treated successfully with radiation therapy and now is from recurrence 5 years after operation. All other cases were treated surgically with abdominoperineal rectal amputations. The result is poor. Local recurrence is common within a or two years after operations. There are only 3 cases who, survive more than 5 years postoperatively.<BR>30 cases of them were mistreated initially under the error of diagnosis such as haemr-rhoidal disease or so on, and lost the chance of early treatment. Tumors advanced beyond the curable resection. It is much demanded not only the extensive resections but also the early correct diagnosis. It is necessary to do pathological examinations immediately when there is any abnormal impression on the course of treatments for anal diseaess.<BR>Inguinal lymphoadenopathy was recognized in 12 cases at: the time of primary operations and in 8 cases of them, malignant metastasis was confirmed histologically. In our cases, there was none that had only groin metasis without intrapelvic lymphnode involvment except a case of malignant melanoma. The groin metastasis appears in most cases subsequently to the intrapelvic lymphnodes involvements, and it means the malignant tumor is extensive advanced.<BR>In our opinions, the value of the prophylactic groin dissections at the time of primary operation is therfore not so high, when appreciated postdissective mobility such as subsequently edema of the lower extremities.
